Our Lewiston office has a 40hr position available for a CCT Social Worker! $3,000 sign on BONUS PURPOSE:
Care Manager Social Worker will, in collaboration with the primary care practice, coordinate services using a systematic and cost-effective approach to provide patient-centered care for a defined population.
The care manager will engage the patient to reduce unnecessary health care services and improve their overall health and well-being.
A specialized focus on housing support will be provided to HOME enrolled patients.
EDUCATION:
Bachelor's degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation or closely related field is required
Experience:
Care management experience, knowledgeable of community resources in Androscoggin, Franklin, Oxford, Penobscot Counties, and Mid-Coast Maine.
Prior knowledge of housing resources and experience working with homeless population a plus.
LICENSURE AND CERTIFICATION:
Current Social Worker licensure or conditional Social Work licensure in the State of Maine.
