Description of Duties: Responsible for the delivery of adult and geriatric patient/resident care through the nursing process which includes Patient/Resident Assessment Planning, Care Delivery/Interventions, Education, Discharge Planning, and Documentation. Professional responsibilities include Quality Improvement activities, Leadership, Growth and Development, and Competency''s. 2 East is a 42 bed Skilled Care Unit which is a Med/Surg Step-Down unit providing services for acute Med/ Surge patients (heart, lungs, strokes, etc) who are not ready to go home from the hospital. Patients come to the Rehab Center from other acute care environments to receive therapy and treatment when hospital care is no longer required. This makes the transition to home or assisted care easier. St. Mary''s d''Youville Pavilion is Maine''s largest long term care facilities.
